Community Health Worker Program

Providing engagement and resource support to Medi-Cal Recipients in Alameda County

Frontline community members that serve as a link between healthcare providers and community members to improve quality of life by addressing social determinants of health.


Services Include:

  • Social Support Assessment - Assessing social health needs and referring to the appropriate community support services.

  • Health Navigation - Assisting individuals in navigating the healthcare system and connecting individuals to social health resources to improve health outcomes.

  • Health Education - Providing education on health issues and advocating for individuals within their communities.

  • Outreach & Engagement - Working actively within the community we serve to notify individuals of available resources and how the community health worker program can help.
